Your proxy holder must attend the meeting. The Secretary must receive this form before your proxy holder votes; please submit it before the meeting begins to allow time for verification. the member in good standing named above, to attend the Annual Meeting of Members on December 6, 2026, and vote on my behalf as my proxy holder. I authorize my proxy holder to vote on the following matters: 1. The proposed amendment to the Articles of Incorporation converting the Society from a nonstock membership corporation to a nonstock directorship corporation. 2. The proposed Amended and Restated Bylaws, under which membership would continue on a nonvoting basis. 3. The related resolutions authorizing the officers to implement the approved changes. 4. Any other business properly brought before the meeting. My proxy holder may decide how to vote on each matter, including voting FOR, voting AGAINST, or ABSTAINING. This proxy is valid only for the December 6, 2026 Annual Meeting and any adjournment of that meeting. It expires when the meeting is finally adjourned. The person appointed above may not transfer this authority to another person. I may revoke this proxy by notifying the Secretary before my proxy holder votes on my behalf.
