A collaboration of the Consulate of Italy in Detroit, the Dante Alighieri Society of Michigan, Campus Salute, Progetto Alfa, Italia America Reputation Lab, and Il Detroiter.
By participating in this voluntary free screening hosted at the Dante Alighieri Society of Michigan, I acknowledge and agree to the following:
Informative Purpose: I acknowledge that the CAMPUS SALUTE screenings are provided for informational and educational purposes only and are not a substitute for professional medical advice, diagnosis, or treatment. I will discuss any screening results or findings with my personal physician or healthcare provider for proper medical evaluation and guidance.
Insurance and Financial Responsibility: The screening is free; no insurance is required.
Medical Record Retention: All personal information will remain confidential and protected in accordance with applicable privacy laws.
Privacy: I am aware that all information collected during the screenings will be kept confidential and used solely for this screening, in accordance with applicable privacy laws.
